This position plays an important role in helping seniors maintain their quality of life, independence, health, and safety while remaining in their homes. The ideal candidate has experience working with older adults, strong communication skills, and the ability to manage multiple priorities in a fast-paced environment. Key Responsibilities Oversee services and provide support to aging-services clients and their families. Conduct in-home intake meetings and complete required documentation. Coordinate client schedules and communicate needs with the scheduling team. Complete quality-assurance visits at required intervals and address concerns or training needs. Train and support caregivers on client-specific care plans, safety, transfers, and proper care techniques. Provide direct care and assistance with Activities of Daily Living (ADLs) as needed. Monitor client health, safety, medications, and implementation of physician orders. Develop, implement, document, and monitor individualized client programs and care plans. Maintain accurate client records, financials, mileage, and required documentation. Communicate client progress, concerns, and changes promptly with the appropriate team members. Build relationships with community partners including PT/OT professionals, hospice, senior centers, and other senior resources. Participate in staff training, educational programs, webinars, meetings, and community marketing events. Assist with licensing and compliance requirements. Maintain HIPAA compliance, confidentiality, and a safe home environment. Perform other duties as assigned. Qualifications 2-year college degree or higher required. Minimum 4 years of experience working with elderly adults. Strong written and verbal communication skills. Excellent organization and ability to multitask. Computer proficiency with email, Microsoft Office, internet, and ability to learn scheduling/documentation software. Ability to maintain financial records and track mileage. Medication certification required. Dependable and responsive to phone calls, texts, and emails. Strong understanding of confidentiality and HIPAA requirements.
