Recipient of 2021 - 2025 Best of Home Care Employer Choice Award Job Overview Griswold Home Care of Fargo is seeking a reliable and caring Client Care Liaison who takes pride in their work and has a strong desire to learn and grow within the company. The ideal Client Care Liaison applicant will possess at least 6 months of experience in general office procedures, proficiency in Microsoft Office programs as well as customer service experience. A background in Health Care or LPN preferred.
Responsibilities:
Customer Service; phone, email, text, etc. Answering phone call and email inquiries from potential Clients to educate them on what services Griswold Home Care can provide. Attending In home Client meetings with potential Clients and Family members to assess Client needs for in home care. Admitting New Clients to in home services; setting up Client Care schedules. Monitoring the Client's Care throughout the duration of services; Care Coordination, administrative assistance and fielding questions from Clients. Monitoring and following up with prospective Clients. Developing Care plans On Call Rotation required (Very Minimal). Other various office duties as assigned. Nursing Duties as delegated by full time staffed RN This position will have limited nursing duties if LPN licensed and will focus on case managment of assigned clients. If LPN Duties would include but not limited to: Training staff, wound care, client assesments, med set-ups & med management, supervision of delegated duties, obtaining Dr. orders,

Requirements:
6 months of experience in an office setting Health Care experience in a direct care or administrative role is highly preferred but not required for this position Caring and compassionate personality On Call rotation required.
