We're seeking an experienced Client Care Manager to join our growing home care team in Monroe Township, NJ. In this role, you'll serve as the primary point of contact for clients and their families, ensuring exceptional service from the initial assessment through ongoing care. You'll collaborate with caregivers, community partners to deliver high-quality, personalized care while helping grow our agency through outstanding customer service and relationship building.   • Conduct in-home client assessments and develop individualized care plans.
  • Coordinate the start of care and ensure services are delivered according to client needs.
  • Perform routine supervisory visits and wellness check-ins.
  • Maintain ongoing communication with clients, families, caregivers, and healthcare providers.
  • Address client concerns promptly and ensure high levels of satisfaction. Care Coordination
  • Collaborate with the scheduling team to ensure appropriate caregiver-client matches.
  • Monitor service quality and recommend adjustments as client needs change.
  • Support caregiver communication and promote positive working relationships.
  • Assist with onboarding new clients and coordinating transitions of care. Qualifications Required
  • High school diploma or equivalent.
  • Bachelor's degree preferred
  • Valid driver's license with reliable transportation.
  • Excellent communication and interpersonal skills.
  • Strong organizational and time management abilities.
  • Proficiency with Microsoft Office (Word, Excel, Outlook).
