• Montclair We're looking for a driven, relationship-oriented Client Care Manager (CCM) to lead sales and referral development for our caregiver services company in Northern New Jersey.

This is a field-based sales role focused on building partnerships with hospitals, rehab centers, assisted living communities, and other healthcare providers—while also converting private-pay leads into long-term clients. If you're someone who enjoys being out in the field, building trust, and closing meaningful business that truly impacts families, this role is for you. What You'll Do Build and grow referral relationships with healthcare facilities and community partners Conduct regular in-person visits (hospitals, rehab centers, senior communities) Generate and convert private-pay leads into active clients Deliver clear, tailored presentations of our services (not one-size-fits-all) Consistently follow up and nurture relationships to drive repeat referrals Track outreach, leads, and performance metrics weekly Collaborate with internal team to ensure smooth client onboarding What We're Looking For Proven experience in sales, business development, or healthcare marketing Strong communication skills and ability to build genuine relationships Comfortable with in-person outreach and field-based selling Self-motivated, organized, and goal-driven Ability to ask for the business and close confidently Valid driver's license and reliable vehicle required Experience in home care, healthcare, or senior services is a plus Compensation & Benefits $3,000/month base salary Commission + performance incentives (uncapped) Clear path to 6-figure earnings based on performance Gas and mileage reimbursement / car allowance provided Marketing expense coverage for client outreach PTO, health benefits, and growth opportunities Why Join 1Heart?
