Various positions Care Coordinator - Duties & Responsibilities Conduct initial client assessments and ongoing reassessments. Develop, update, and maintain individualized care plans. Communicate care-plan details and changes to caregivers promptly. Serve as the primary point of contact for clients and families. Monitor client condition and ensure needs are being met. Document all client interactions, updates, and incidents. Coordinate with Scheduler to ensure proper staffing. Provide guidance to caregivers regarding client needs and expectations. Respond to client concerns and escalate issues to Supervisor when needed. Ensure compliance with state regulations and agency policies. Participate in quality assurance and audit-readiness activities. Scheduler - Duties & Responsibilities Create, maintain, and update the master caregiver schedule. Assign caregivers based on skill, availability, and client needs. Manage call-outs, emergencies, and last-minute schedule changes. Communicate shift assignments and updates to caregivers. Track attendance, tardiness, and schedule adherence. Ensure all shifts meet regulatory and agency requirements. Maintain accurate documentation of schedule changes and coverage gaps. Coordinate with Care Coordinator regarding client needs and staffing. Provide daily schedule reports to Supervisor/Administrator. Caregiver - Duties & Responsibilities Provide personal care (bathing, grooming, dressing, toileting). Assist with mobility, transfers, and fall-prevention. Perform light housekeeping, meal prep, and companionship. Follow the care plan and complete all assigned tasks. Document services provided during each visit. Report changes in client condition immediately. Maintain client safety, dignity, and confidentiality. Use proper infection-control and safety procedures. Maintain professional boundaries at all times. Communicate effectively with Care Coordinator and Supervisor. Trainer - Duties & Responsibilities Conduct new-hire orientation and skills training. Provide hands-on demonstrations of caregiving techniques. Evaluate caregiver competency and document results. Offer refresher training and corrective coaching as needed. Ensure all training meets state and agency standards. Maintain training logs, sign-in sheets, and compliance records. Identify skill gaps and recommend additional training. Support caregivers during onboarding and early employment. Report training progress and concerns to Supervisor/Administrator. Supervisor - Duties & Responsibilities Oversee daily operations and support all staff. Supervise Care Coordinators, Schedulers, Trainers, and Caregivers. Respond to incidents, emergencies, and client concerns. Conduct performance reviews and corrective actions. Ensure compliance with state regulations and agency policies. Review documentation for accuracy and completeness. Assist with client assessments and care-plan oversight. Provide leadership, guidance, and conflict resolution. Maintain audit-ready records and support quality assurance. Communicate regularly with Administrator/Owner regarding operations. HR Assistant - Duties & Responsibilities Assist with recruiting, interviewing, and onboarding new employees. Maintain employee files and ensure all compliance documents are current. Track training, certifications, background checks, and renewals. Support disciplinary processes and documentation. Assist with payroll-related paperwork and employee communication. Maintain confidentiality and proper record-keeping. Ensure all HR processes follow agency policies and state regulations. Provide administrative support to HR Manager/Administrator. Help coordinate staff meetings, trainings, and compliance deadlines.
