$29.14 - $30.89 Job Summary Heavenly Hands is seeking a proactive, compassionate, and detail-oriented Service Coordinator to join our growing team. The Service Coordinator plays a critical role in ensuring clients receive high-quality services by coordinating care, maintaining accurate records, communicating with providers and families, and supporting compliance with agency and regulatory requirements. The ideal candidate is organized, customer-focused, and able to manage multiple responsibilities in a fast-paced environment while maintaining professionalism and confidentiality. Responsibilities Coordinate services for assigned clients and ensure service plans are implemented effectively. Maintain regular communication with clients, families, caregivers, and community partners. Schedule and coordinate appointments, assessments, and service visits. Monitor service delivery and address concerns in a timely manner. Maintain accurate and up-to-date client records and documentation. Enter and manage data within agency systems and databases. Assist with client intake, assessments, and service plan updates. Ensure compliance with agency policies, Medicaid requirements, and applicable regulations. Collaborate with interdisciplinary team members to support client goals and outcomes. Prepare reports and track service-related metrics as needed. Participate in staff meetings, training sessions, and quality improvement initiatives. Qualifications Associate's required , Bachelor's degree preferred in Psychology, Human Services, Social Work, Healthcare Administration, or a related field preferred. Previous experience in service coordination, case management, healthcare, home care, or human services preferred. Strong organizational and time-management skills. Excellent verbal and written communication skills. Proficiency in Microsoft Office Suite and electronic record systems. Ability to manage multiple priorities while maintaining attention to detail. Professional phone etiquette and customer service skills. Ability to work independently and as part of a team. Valid driver's license and reliable transportation preferred.
