In this role, you will serve as the single point of contact for families navigating care for their loved ones—guiding them through care consultations, developing personalized service plans, and ensuring every client receives the exceptional support Home Instead is known for. Who We're Looking For The ideal candidate is a motivated, organized, and compassionate professional who welcomes the responsibility of supporting older adults and their families through personalized care coordination. As the primary point of contact for both new and existing clients , you will guide families through care decisions while ensuring every client receives high-quality, personalized support. You will conduct care consultations with families seeking assistance for their loved ones, develop customized care plans, and coordinate services with our Care Professionals to ensure exceptional care delivery. Through ongoing communication and quality assurance visits, you will maintain trusted relationships with clients and families while ensuring their needs continue to be met. A genuine passion for helping others, strong communication skills, and the ability to manage multiple priorities will be key to success in this role. What You'll Do Make an
Impact:
Serve as the primary advocate and point of contact for clients and their families, building trusted relationships and ensuring each client receives compassionate, high-quality care.
Guide Families Through Care Decisions:
Conduct care consultations with prospective clients and families to assess needs, develop personalized care plans, and guide them through the process of beginning services.
Coordinate Exceptional Care:
Collaborate with Care Professionals, schedulers, and office team members to ensure seamless service delivery. Coordinate introductions between clients and Care Pros to ensure a strong start to services.

Conduct ongoing quality assurance visits and maintain regular communication with clients and families to ensure care plans remain effective as needs evolve.

Document care consultations, service plans, and quality assurance visits within agency management systems while ensuring compliance with company policies and documentation standards. What Makes You a
Great Fit Ensure Quality & Satisfaction:
Maintain Client Records & Compliance:
Bachelor's degree in Healthcare Administration, Social Work, Gerontology, Communications, or similar field (or equivalent experience preferred ). Experience working directly with older adults and their families , particularly in care planning, patient services, or client advocacy. Prior experience in healthcare, senior care, hospice, home health, social work, or care coordination ( preferred) . Excellent written and verbal communication skills. Well-organized with strong time and activity management skills. Strong problem-solving abilities and the ability to manage multiple clients and priorities. Able to work independently as well as on a team. Valid driver's license and reliable transportation for client visits as needed. Ready to Make a Difference?
