Overview The Apartment Services Coordinator provides essential, compassionate, and personalized care to residents within an apartment setting. This role provides day-to-day oversight and supervision to Supported Living Caregivers, ensuring that the daily physical, emotional, and social well-being of residents is maintained.
Key Responsibilities:
Health Monitoring and Documentation Observation:
Monitoring and reporting any changes in a resident's physical, mental, or emotional condition to the Apartment Services Coordinator .
Medication Management:
Providing timely and thorough medication monitoring support.
Doctor Appointments:
Attending scheduled appointments and providing Apartment Services Coordinator with updates.
Documentation:
Completing accurate and timely records of all care provided, observations made, and incident reports using the designated electronic or paper system. Apartment and Personal Care Support The Caregiver is responsible for providing guidance, support, encouragement and prompting for daily living skills.
Tidiness:
Ensuring the resident's apartment is safe, tidy, and organized.
Laundry:
Ensuring the resident's laundry is maintained.
Nutrition:
Ensuring the resident has access to nutritious food and is eating regular meals. Helping with meal planning and grocery shopping.
Social and Emotional Support Engagement:
Encouraging and assisting residents to participate in scheduled social activities and programs.
De-escalation:
Providing active listening to resident concerns, and addressing day to day needs (i.e. minor roommate concerns and shopping requests).
Transportation:
Assisting with transportation as needed including, grocery shopping, doctor appointments, work transportation and bus training. Communication Attend and participate in quarterly meetings and as requested. Regular communication and collaboration with caregivers, Coordinators and Supervisor. Comply with all Electronic Visit Verification (EVV) steps. Act as a primary contact for members of client care teams. Read and complete house logs, resident documentation, incident reports and schedules. Carry and answer a company cell phone during shift and while on call. Collaborate and provide support to other Apartment Services Coordinators. Report any issues, concerns, or maintenance needs immediately to the Supervisor. Conduct yourself professionally at all times, give and receive feedback appropriately. Supervision and Guidance Provide day to day guidance to Supported Living Care Givers. Address needs of Supported Living Caregivers as they arise. Provide timely and thorough medication monitoring support to clients. Complete monthly apartment checks or more frequently as needed. Office and Other Duties Collaborate with Director of Apartment Services on monthly lunches and birthday gifts (client/staff) Coordinate all vehicle related maintenance and assign who will take the vehicle. Order office related supplies and maintain an accurate inventory and stock. Ensure all client appointments are scheduled, rescheduled, or canceled as required. Coordinate client co-payments for appointments with Apartment Services Supervisor or Director. Assign daily tasks to staff and ensure accountability and follow through. Ensure all company vehicles are maintained, clean and fueled. Perform monthly apartment checks. Assist with coordinating work and leisure transportation (bus training/transport services). Assist with meal planning. Other duties as assigned by the Apartment Services Supervisor or Director of Apartment Services. Schedule Requirements Must work a minimum of a 40 hour week. Must be able to work at least one weekend shift per month. Must be able to rotate on call responsibilities throughout the week. Qualifications Must have and maintain a valid driver's license in good standing and be able to be an approved driver under the company's liability insurance coverage. Must have a minimum of a high school diploma or equivalent. Must be able to give and receive feedback in a professional manner. Must be proficient in reading, writing and speaking English. Working Conditions and Physical Demands Must be able to lift at least 25lbs. Must be able to sit for extended periods in an office environment or in a vehicle. Must be able to traverse in home, office and community settings.
Pay:
$18.00 - $19.00 per hour Expected hours: 40.0 - 50.0 per week
