Position Purpose As a Care Coordinator, you play a vital role in delivering exceptional, client-focused care to seniors, veterans, and their families. This position is responsible for coordinating client services, maintaining care plans, supporting scheduling operations, conducting quality assurance visits, and ensuring effective communication between clients, families, caregivers, referral sources, and the Veterans Administration. Job Overview We are seeking a compassionate and organized Client Care Coordinator to help ensure exceptional care for seniors and veterans in our community. This role is responsible for coordinating client services, maintaining care plans, conducting assessments, supporting scheduling needs, and serving as a key point of contact for clients, families, caregivers, and healthcare partners. Key Responsibilities Client Care Coordination & Quality Assurance Care Plan Oversight :
Create, update, and review individualized care plans. Ensure care plans reflect current client conditions, preferences, and goals.
Client Communication :
Maintain regular communication with clients and families to evaluate satisfaction and update care needs.
Caregiver Collaboration :
Communicate updates and changes to care plans with assigned caregivers in a timely and clear manner.
In-Home Assessments :
Conduct evaluations to assess care needs, home safety, and overall wellness. Document and implement any required updates to care plans.
Quality Checks :
Assist with periodic client follow-ups to ensure care quality and address any issues or concerns proactively.
Documentation & Reporting System Accuracy:
Maintain accurate and timely records inAxisCare .
Scheduling Coordination Schedule Maintenance :
Support schedule creation and shift updates to ensure optimal coverage.
Open Shift Coverage :
Monitor AxisCare for shift gaps and address clock-in/clock-out compliance with caregivers.
On-Call Support :
Participate in a rotating on-call schedule to manage urgent scheduling needs during evenings and weekends.
Cross-Team & Administrative Support Cross-Training :
Serve as emergency back-up for other administrative team roles.
Phone & Office Management :
Answer calls professionally; maintain office supplies and client documentation.
Client Relations :
Address client requests and concerns, escalating as appropriate. Veteran Services Coordination Coordinate services for Veterans receiving care through the Veterans Administration Communicate with VA social workers, case managers, clients, families, and caregivers Monitor authorization periods and service utilization Maintain required VA documentation and reporting Assist with care transitions, reassessments, and service updates as needed
Performance Monitoring :
Track attendance and punctuality; support coaching efforts as directed by leadership.
Reporting :
Analyze care-related data and identify opportunities for improved service delivery. Experience - Required Minimum 2 years of experience as a Care Coordinator, Client Care Coordinator, Scheduler, Staffing Coordinator, or similar role in home care, home health, hospice, or senior services Previous hands-on experience using AxisCare software Experience coordinating services for Veterans through the Veterans Administration Homemaker/Home Health Aide (H/HHA) Program and/or Respite Care Program Strong understanding of caregiver scheduling, shift management, and client service coordination Excellent verbal and written communication skills Strong organizational skills with the ability to manage multiple priorities in a fast-paced environment Proficiency with Microsoft Office and web-based software applications Ability to maintain confidentiality and handle sensitive information professionally Valid California driver's license and reliable transportation Ability to pass DOJ/FBI background screening Work Conditions & Schedule Monday-Friday, 7:30 AM-4:30 PM PST (in-office) Rotating on-call schedule (evenings/weekends) Stationary 80% of the workday Must work effectively in a fast-paced environment
Pay:
$26.00 - $28.00 per hour
