Coordinate respiratory and durable medical equipment (DME) needs for patients preparing for discharge. Facilitate timely delivery and setup of necessary medical equipment at hospitals and patient homes. Educate patients and caregivers on proper equipment use, safety, and maintenance. Collaborate closely with case managers, social workers, nurses, and physicians to ensure smooth patient transitions. Maintain accurate referral documentation, insurance verification, and activity records. Troubleshoot equipment issues and assist hospital staff with service-related questions. Participate in quality improvement initiatives and healthcare compliance programs. Travel to patient homes for equipment setup and follow-up as needed.

What You Need Required:

High school diploma or GED Valid driver's license with a clean driving record Strong communication and relationship-building skills Ability to work effectively inside a hospital environment Strong organizational skills and attention to detail

Preferred:
