At Crossroads Community, Inc. (CCI), we are committed to helping individuals with behavioral health needs build meaningful, independent, and productive lives. Our team works every day to support recovery, resiliency, and community integration through compassionate, person-centered services. We are currently seeking a Client Support Specialist to join our Psychiatric Rehabilitation Program team. This role is ideal for someone who enjoys working directly with people, building supportive relationships, teaching life skills, and helping individuals reach personal goals in both program and community settings. Position Summary The Client Support Specialist provides rehabilitation and recovery-focused support services to adults receiving behavioral health services. Staff work collaboratively with clients to identify strengths, develop goals, connect to community resources, and build skills that support independent living and overall wellness. This position includes a combination of direct client interaction, documentation, coordination of care, transportation assistance, and community-based support. Primary Responsibilities Support individuals in identifying personal goals, strengths, and service needs Assist with development and implementation of Individual Rehabilitation Plans (IRPs) Provide coaching and support related to daily living skills, coping strategies, communication, socialization, and community integration Facilitate individual and group-based rehabilitation activities Monitor and document client progress toward established goals Coordinate care and communicate with providers, supports, and caregivers as appropriate Maintain accurate and timely electronic documentation in accordance with agency, regulatory, and accreditation standards Provide transportation for clients to appointments, community resources, and other approved activities when necessary Participate in team meetings and ongoing collaboration with clinical and rehabilitation staff

Qualifications:
High school diploma or equivalent required Associate's or Bachelor's degree in Behavioral Health, Psychology, Social Work, Human Services, or related field preferred Previous experience working in human services or behavioral health is highly valued strong interpersonal and communication skills basic computer proficiency and ability to complete electronic documentation
Additional Requirements:
Ability to pass agency background screening Valid driver's license with acceptable driving record Ability to safely transport clients when required Access to a vehicle for work-related duties if needed Proof of minimum automobile liability insurance coverage of 100/300/50
