Job Description

Company Overview AlphaCare Home Health is a licensed Northern Virginia home-care organization dedicated to helping individuals live safely and comfortably in their own homes. Job Summary We are seeking a dynamic and results-driven Home Care Community Liaison & Client Intake Specialist to help grow our referral network and guide prospective clients from their initial inquiry through the administrative start-of-care process. The ideal candidate will have experience in home care, senior care, healthcare outreach, client intake, admissions, sales or business development. This person must be comfortable developing community relationships, following up consistently with leads, conducting in-home and virtual consultations and producing measurable results. This is a full-time, field-based and hybrid administrative position. It is not a fully remote marketing role. Schedule and Location Full-time, W-2 position 40 hours per week Standard schedule: Monday through Friday, 8:30 AM-5:00 PM Approximately three field days and two administrative or remote-work days per week Regular travel throughout Northern Virginia Occasional evening or weekend community events may be required with advance notice Duties Community Outreach and Business Development Develop and implement outreach strategies to connect with hospitals, rehabilitation centers, skilled-nursing facilities, assisted-living communities, senior centers, physician practices, hospice organizations, geriatric care managers, elder-law attorneys, faith-based organizations and other community partners. Build and maintain strong referral relationships through regular in-person visits, telephone calls, emails, presentations and professional follow-up. Identify new opportunities for home care referrals and service growth. Conduct informational presentations to educate referral partners and community organizations about AlphaCare's services. Represent AlphaCare at community events, health fairs, networking events and professional meetings. Manage an assigned territory and maintain an organized schedule of referral-partner visits. Track which outreach activities and referral relationships generate qualified leads and new clients. Lead Follow-Up and Client Intake Respond promptly and professionally to prospective clients and referral partners. Follow up consistently with leads by telephone, text and email. Conduct free in-home and virtual consultations with prospective clients and their families. Gather nonclinical information regarding care needs, requested schedules, service location, anticipated start date and family expectations. Explain AlphaCare's services, rates, deposits, scheduling requirements and applicable policies. Assist families with administrative admission documents and required signatures. Coordinate with AlphaCare's RN for required nursing assessments, clinical determinations and plans of services. Collaborate with nursing, scheduling and caregiver-placement staff to support a smooth start of care. Maintain accurate documentation of leads, consultations, admissions and referral activity. Ensure every lead and referral partner has a documented status and next action. Performance and Reporting Use AlphaCare's lead-tracking system to document outreach, interactions and follow-up tasks. Submit weekly reports covering leads, response times, referral visits, consultations, admissions, client starts and service hours generated. Meet established outreach, consultation, referral-development and client-conversion goals. Participate in 30-, 60- and 90-day performance reviews and quarterly reviews thereafter. Qualifications Two or more years of experience in home care, senior care, healthcare outreach, admissions, client intake, outside sales, account management or business development is preferred. Home-care or senior-services marketing experience is strongly preferred. Existing professional relationships within the Northern Virginia healthcare or senior-services community are highly desirable. Demonstrated success generating referrals, developing professional relationships or converting inquiries into clients. Excellent communication, presentation and interpersonal skills. Strong follow-up, organization, time-management and documentation skills. Comfortable discussing private-pay services and pricing with prospective clients. Comfortable conducting consultations inside prospective clients' homes. Ability to work independently, manage an assigned territory and meet measurable performance expectations. Proficiency with Microsoft Office, email, calendars, CRM systems and electronic documentation. Valid driver's license, reliable personal transportation and current automobile insurance are required. Must be willing to travel throughout Fairfax, Loudoun, Arlington, Alexandria City and Prince William. Must successfully complete all required employment and background screenings. A Virginia RN license is welcome but is not required. Pay:
$60,000.00 - $65,000.00 per year
