5/4-6/30 - Monday - Thursday, 8am-4pm

Setting:

Clinic Procedures/Cases:

98% of patients are adults.

Procedures needed:

All complex surgical procedures.

Credentialing Timeframe:

~1-2 days

Requirements:

Active State License, Drug Enforcement Administration (DEA), Basic Life Support (BLS), Cardiopulmonary Resuscitation (CPR), and use of Dentrix Electronic Medical Record (EMR), Controlled Substance Registration (CSR) #
