Since 1979 TRAC Services for Families has worked so that no child will have to face a life without permanency. TRAC was founded by adoptive parents and professionals and established as a volunteer staffed project of the Junior League of Pittsburgh. Today, TRAC is a full service agency that serves as the central resources for permanency information, adoption and foster family recruitment, placement of special needs children, post-permanency support, outpatient therapy, education, and referral services throughout Allegheny and its contiguous counties. Adhering to the permanency standards of the Child Welfare League of America, TRAC is a multi-service agency serving children who cannot remain with their birth families and the resource families who provide these children permanence. TRAC works with public and private agencies to implement cooperative, cost effective programs to meet the placement and post-placement needs of these children. Basic Function To complete high quality and thorough SWAN child and family profiles referred to TRAC by the Statewide Adoption Network (SWAN) and assigned by the Program Supervisor. Responsibilities Strive to complete each referred profile within the SWAN allotted time frame of 90 days with the understanding that delays may occur. This includes documentation of all contacts, such as phone and email contact with county workers, collateral parties and families, in depth file reviews, home visit with child and caregiver, meetings/phone calls with therapists, foster care workers and others involved in the case. All contacts must be included in the final child profile. Maintain contact with the referring county caseworker to keep them advised of the progress of the profile. Submit the completed child profile to the Coordinator via email which includes a current color photograph of the child profiled. Complete the SWAN benchmarks for each completed profile, including time tracking, on the SWAN portal. Responsible to attend all agency required meetings such as staff meetings, team meetings, trainings and other TRAC events as determined by upper management. Experience in adoption and or foster care a plus. A combination of education and experience may be accepted as approved by the CEO.
