Position Description:
The Designated Manager (DM) is responsible for the overall management, quality, and regulatory compliance of the organization's Minnesota 245D-licensed Home and Community-Based Services (HCBS) programs. As the DHS-recognized Designated Manager, this leader ensures adherence to Minnesota Statutes, Chapter 245D, while fostering a culture centered on service excellence, accountability, and person-centered supports. The DM collaborates closely with executive leadership to design and improve operational systems, supervise Designated Coordinators and Program Supervisors, and maintain full regulatory readiness. This role is key in driving organizational growth, operational integrity, and exceptional service outcomes Key Responsibilities Regulatory Compliance & Oversight Serve as the Designated Manager per Minnesota Statute 245D.081, responsible for daily oversight of program operations. Ensure ongoing compliance with 245D, 245A, and related Minnesota rules, including service delivery, documentation, staff training, and incident reporting. Oversee licensing renewals, variances, audits, and investigations. Maintain and implement company policies, procedures, and corrective action plans in accordance with DHS and MDH requirements. Conduct regular site visits and internal audits to verify compliance and quality standards. Develop new initiatives to support the program and organization as needed to help with improvement areas as needed Making sure that this correlates with the county Coordinated Support Plan (CSP), Coordinated Services and Support Plan (CSSP) and/or the Individual Service Plan (ISP) ▪ Self-Management Assessment Plan (SMA) ▪ Individual Abuse Plan (IAPP) ▪ Service Supports and Outcomes as determined by the individual and team members ▪ Person-Centered Profile/Planning Tools ▪ Other forms as requested and needed Review and complete other needed forms as needed This could include : ▪ Medication Error Reports ▪ Seizure Reports ▪ Medication Records ▪ Health Progress Notes ▪ Daily Progress Notes ▪ Individual Outcomes ▪ Individual Finances ▪ Communication Book ▪ Other areas/forms as needed Organize the program
  • Ensure that program activities operate within the policies and procedures of the organization This includes following up when activities are not completed as scheduled and letting staff know the importance of completing scheduled activities
  • Develop forms and records to document program activities and daily outcomes and progress for all individuals served within the home
  • Communicate effectively with all team members both internally and externally This includes : ▪ Owners ▪ Designated Managers ▪ Program Supervisors (as applicable) ▪ Direct Support Professionals ▪ Individuals ▪ Guardians ▪ Family Members ▪ Case Managers ▪ Day Programs ▪ Medical Personnel ▪ Others as determined by teams
  • Attend team meetings and appointments for individuals as needed These may be on site or off site depending on team preference Quality Assurance & Reporting Develop and oversee internal quality assurance processes, including review of service delivery records, outcomes, and incidents.
Submit required reports to DHS and other oversight bodies in a timely and accurate manner. Analyze data to identify trends, risks, and opportunities for improvement. Report concerns, risks, and trends to leadership. Staff the program
  • In consultation with the HR Director, the needs and request.
  • Implement the human resources policies, procedures and practices of the organization that are shared within the organization and programs Consult with Program Supervisors and Direct Support Professionals when these are not being followed
  • Implement a performance management process for all program staff This includes completing a yearly performance evaluation for each staff member at the site
  • Ensure that all program staff receive an appropriate orientation to the organization and the programs Lead the program
  • Ensure all staff members receive orientation and appropriate training in accordance with organizational standards
  • Supervise program staff by providing direction, input and feedback Both onsite and during quarterly staff meetings
  • Liaise with other managers to ensure the effective and efficient program delivery is consistent throughout the organization
  • Coordinate the delivery of services among different program activities to increase effectiveness and efficiency Control the program
  • Write reports on the program for management and individual teams This includes quarterly reports, financial reports, and annual paperwork
  • Ensure that the program operates within the approved budget This includes reviewing expenses and asking questions when needed with regards to overuse or underuse of the budget
  • Ensure that all financial records for the program are up to date and accurate This includes both business financial records as well as individual financial records Qualifications Education (Preferred) Associate's degree in a related subject Bachelor's Degree in a related subject No more than 5 years of experience Licensing
  • Valid driver's license with an acceptable driving record It must be comfortable transporting individuals in a company vehicle (van) to and from community activities and appointments as needed Must be willing to follow all traffic laws, including the use of a seat belt, following the speed limit, no use of phone for texting purposes, etc.
Any moving violations obtained throughout employment must be discussed with Human Resources and/or Owners to assure that you are still eligible to operate a company vehicle
  • Reliable vehicle and valid proof of liability insurance If a company vehicle is not available, it may be requested that you use your own personal vehicle Insurance must be submitted each time it expires and remains current and up to date within the employee's personnel file
  • Ability to pass a background check by the Bureau of Criminal Apprehension and the Department of Human Services
  • First Aid, CPR and AED Certification This is offered to all employees to always keep current throughout employment
  • Medication Administration Knowledge, skills and abilities
  • Previous residential setting experience
  • Knowledge of program management
  • Knowledge of client groups and/or issues related to the program area
  • Ability to react quick in critical situations

                    • 3 to 5 years' experience in a related field (preferably a supervisory position) Working Conditions
                    • Designated Managers will work both in the office and in their assigned residential setting(s)
                    • Time should be utilized as evenly as possible amongst programs. For example, if you are managing 3 programs, you should average your time about your time at each program, give or take on the needs of the program
                    • Designated Managers must have flexibility with scheduled hours in times of need with regards to filling open shifts, crisis situations, etc. This at times may cause overtime which can and will be approved when needed
                    • Calls, texts, and emails may occur during unscheduled times.
                    It is not required that you respond to these matters while not working, however if you are not able to, have a backup plan for who can assist with these matters during that time. However, in an emergency, crisis, maintaining a site that is having trouble or training of a supervisor, your attention and support is appreciated. If the interference is excessive or a hindering factor; compensation will ensue.
                    • Designated Managers will share responsibility of being on-call for the agency along with other team members.
                    On Call responsibilities include carrying the on-call phone for one week, 24 hours a day, responding to all calls within a timely fashion, and problem solving as needed. This could include the need to go into any program and assist as needed during unscheduled work time. You are also required to drop by three designated sites on a Saturday and/or Sunday during the time that you are on-call
                    • Designated Managers are expected to use and share their work calendars with times that they are both available and unavailable.
                    o Scheduled time could include site visits, appointments, meetings, etc. o Any time not worked during the scheduled work week must be requested and approved off by both owners via email and/or text message. ~ Requests for time off should be requested in advance, when possible and be in the beginning of your work day, include the whole work day, or be at the end of your work day to allow a steady amount of time to be worked without personal interruption, if and when possible. o When you are off, please include in your calendar that you are off, but designate who is covering for you should something come up
                    • Participation at Libra events outside of scheduled work time is voluntary.
                    Some Libra events may be paid depending on the event
