JOB DESCRIPTION
Diabetic visit nurse, RN or
LPN JOB SUMMARY
: A qualified Licensed Practical/Vocational Nurse administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Registered Nurse. Services are furnished in accordance with Agency policies.
QUALIFICATIONS
: Graduate of a state approved school of practical (vocational) nursing and currently licensed in the state(s) in which practicing. Minimum of one (1) year experience in nursing, preferred. Acceptance of philosophy and goals of this Agency. Ability to exercise initiative and independent judgment.
RESPONSIBILITIES
: Understands and adheres to established Agency policies and procedures. Following MD eval and treat orders, RN develops the plan of care and determines the amount and type of nursing care required by each patient. RN completes and submits OASIS (Outcome and Assessment Information Set) assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition in accordance with Agency-defined time limits. Maintains an up-to-date Care Plan and medication profile in the patient's electronic chart with regular collaboration with the MD. This care plan includes a detailed outline of skilled nursing daily diabetic care required for each patient. Provides skilled nursing services in accordance with the plan of care. Educates the patient and family/significant to others in diabetic management and other skilled nursing or related needs. Observes any abnormal signs and symptoms, reactions to treatments, including drugs, and changes in the patient's physical or emotional condition to provider and care team. Completes diabetic visits as assigned within 1 hour of scheduled time. Completes charting real time according to company policies (see visit expectations). Maintains accurate and complete diabetic binder kept at each patient's residence to include a face sheet, plan of care with medication list, and blood sugar log. Develops a working relationship with ALF staff following their policies and procedures - always completing outside agency visit sheets, updating changes in condition and providing a current plan of care. Understands ADA guidelines and has a comprehensive understanding of prescribed insulin onset, peak and duration, in addition to appropriate management of hypo and hyper glycemia and target A1C. Refers to Physical Therapist, Speech Language Pathologist, Occupational Therapist and Medical Social Worker for those patients requiring specialized skills. Attends IDG weekly, QAPI quarterly, continuing education as assigned, and any scheduled nursing education or in-services provided. Participates in weekend and evening rotation for diabetic visits as assigned.
Diabetic visit expectations:
Routine visit length 20-30 minutes. It is never less than 20 minutes. Real time clock in and out at each visit, obtaining a patient or caregiver signature. Visit time is made within the same hour each day. Comprehensive visit, 30-45 minutes, and includes a full assessment. Unique narrative notes for each visit. No copy or paste. Barrier to independence identified in each note. Full set of VS and BS at each visit Never administer short-acting insulin without personally checking BS and filling out binder to ensure no duplication of visits Full comprehensive assessment of each patient weekly - typically done by Diabetic Team Lead Weekly foot checks Routine or missed visit charting submitted the same day Admission, recert, or Resumption visit charting completed within 24 hours Consents submitted same day as admission
WORKING ENVIRONMENT
: Works indoors in Agency office and patient homes and travels to/from patient homes.

: Ability to perform the following tasks if necessary: Ability to participate in physical activity. Ability to work for extended period of time while standing and being involved in physical activity. Heavy lifting. Ability to do extensive bending, lifting, and standing on a regular basis.
