POSITION PURPOSE
Coordinates and delivers specialized nursing care to patients receiving home dialysis therapies, with primary responsibility for patient training, ongoing assessment, education, and clinical support. Functions as a key clinical resource to patients and the interdisciplinary care team, promoting safe, effective care ensuring compliance with
CMS, ESRD

MINIMUM EDUCATION REQUIRED
Associate Degree in Nursing required. Bachelor of Science in Nursing preferred.
MINIMUM EXPERIENCE REQUIRED
Minimum of 12 months Registered Nurse (RN) experience required. Prior experience in home dialysis therapies preferred (peritoneal dialysis [PD] and/or home hemodialysis [HHD]).
LICENSES / CERTIFICATIONS REQUIRED
Current BLS and/or CPR certification from the American Heart Association or American Red Cross or the ability to obtain certification within 90 days of hire. Licensure to be eligible to practice as a Registered Nurse or Graduate Nurse in the State of Wisconsin.
KNOWLEDGE, SKILLS & ABILITIES REQUIRED

Conditions for Coverage related to home dialysis therapies. Ability to read, write, hear, speak and comprehend the English language. The physical, technical and cognitive ability as well as the manual dexterity, to use patient care equipment. Basic computer skills.
PRINCIPLE ACCOUNTABILITIES AND ESSENTIAL DUTIES
Coordinates and oversees the day-to-day operations of the dialysis home therapies training facility. Provide training and education to patients, families, and designated support persons for home dialysis therapies. Performs ongoing patient assessments to monitor progress and competency in managing hemodialysis therapies. Participates in a scheduled on-call rotation to provide clinical support to home dialysis patients during off hours. Provides training in self-cannulation techniques, (AVF, AVG, Buttonhole, and catheter access), including access assessment and care. Provides training on peritoneal dialysis (PD) cycler use, including connection and disconnection procedures. Provides education and training on emergency management, including emergency disconnects, blood leaks, air embolism risks, power failures, and emergency evacuation. Provides training on operation and troubleshooting of dialysis machines, PD cyclers, and related equipment. Coordinates and oversees source water quality and microbial testing in collaboration with vendors and contracted service providers. Assesses patient home environments for safety, infection control, and therapy readiness prior to initiation and throughout ongoing therapy. Ensures documentation for home dialysis therapies meets CMS and organizational requirements. Monitors, tracks, and reports home dialysis quality indicators as part of QAPI activities. Utilizes remote monitoring platforms to assess treatment adherence, troubleshoot issues, and intervene as clinically indicated. Provides training on specimen collection, labeling, centrifuge use, and proper handling and transport of laboratory specimens.
