Job Description
Job Summary We are seeking a compassionate and dedicated Direct Care Professional to provide exceptional support and care to individuals with diverse needs. In this vital role, you will be the primary caregiver, fostering a safe, respectful, and nurturing environment. Duties Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor residents' health by taking vital signs, observing patient conditions, and reporting changes promptly Support medication administration in accordance with care plans and HIPAA regulations to ensure privacy and safety Prepare nutritious meals tailored to individual dietary needs and assist residents during meal times Maintain a clean and safe environment through regular cleaning, sanitation, and organization of living spaces Use specialized equipment such as Hoyer lifts or resident's to safely transfer individuals with mobility challenges Document care activities accurately in resident records, including progress notes and incident reports Engage residents in social activities and provide companionship to promote mental well-being Follow all safety protocols including CPR, First Aid, and emergency procedures to ensure resident safety at all times Collaborate with team members, healthcare professionals, and family members to develop and implement personalized care plans Requirements Prior experience working in assisted living facilities, group homes, nursing homes or home & community care settings preferred Knowledge of dementia care, Alzheimer's care, memory care, hospice care, or working with individuals with developmental disabilities or autism is highly desirable Certification in CPR and First Aid is required; additional certifications such as HIPAA training are a plus Ability to lift heavy objects safely and perform physical tasks required for patient monitoring and transfers Excellent communication skills to effectively observe residents' needs and document care activities accurately Compassionate attitude with a strong commitment to providing respectful resident support Flexibility to work various shifts including evenings, weekends, or overnight hours as needed Ability to maintain confidentiality in accordance with HIPAA regulations while handling sensitive information Join our team as a Direct Care Professional where your dedication transforms lives!
