Job Summary:
The Direct Support Professional (DSP)
  • Bridges provides exceptional care and support to individuals with disabilities in a compassionate and dedicated manner. In addition, the DSP
  • Bridges will assist day program management in the planning and implantation of programs, activities, work opportunities and community integrated outings. The DSP
  • Bridges is responsible for assisting with daily living activities, transportation, meal preparation, medication reminders and administration (with training and certification), and ensuring documentation is recorded daily via electronic participant record software.
This role requires empathy, patience, and a commitment to motivating participants to participate in meaningful day activities while maintaining compliance with participant Individual Service Plans (ISPs) and promptly documenting any adverse behaviors or incidents for proper intervention.
Essential Functions:
Collaborate with the Quality Improvement Assistant Collaborate with Residential Management Staff Ensure monthly advocacy meetings are offered to all participants Ensure that group calendars are updated, and that the activity schedule is followed. Follow Daily Schedule and rotate attention to all participants throughout the day. Follow standards of employee conduct Ensure all outcomes, supports, tasks and progress notes are implemented and entered accordingly. Verify that all necessary supplies are readily available for upcoming activities. Monitor lunchtime procedure to ensure all guidelines are being followed Monitor bus time procedure to ensure all guidelines are being followed Ensure that documentation, participant personal care, activities, etc. are all done as required. Comply with staff lunches and break schedule. Assist participants in personal care tasks such as toileting/changing, eating, washing, and grooming. Support participants in activities of daily living, encouraging independence and participation in meaningful day activities. Provide transportation for community integrated outings and recreational activities. Administer medication following care plans and medication protocols, ensuring accurate documentation. Medical certification required. Record daily activities, participant progress, and any adverse behaviors or incidents in electronic participant record software for compliance. Create a safe and supportive environment for individuals with disabilities. Collaborate with families, healthcare professionals, home staff, and QIDPs to ensure coordinated care and adherence to ISPs. Perform other duties as assigned or directed
Job Qualifications:
Prior experience as a DSP. High school diploma or GED required; relevant certifications or training a plus (Med Cert, First Aid, CPR, etc.) At least 21 years of age to be a driver per insurance guidelines. If not a driver, 18 years of age minimum. Valid driver's license and a good driving record with less than 6 points on license. Ability to pass drug test, physical, and all pre-employment background checks. Strong written, verbal, and interpersonal skills. Ability to meet physical demands of the role, including lifting and transferring individuals. Compassion, patience, and a genuine desire to enhance the lives of others. Education and Experience High School Diploma or GED. Physical Demands The physical demands described are representative of those that each employee must meet to perform the essential functions of this job. These physical demands are representative of the physical requirements necessary for an employee to successfully perform the essential functions of the Bridges program. The ability to lift/pull/carry various objects of 20-50 pounds frequently. The ability to lift, transfer and help participants in any way that may require lifting at least 60 pounds. Handle different forms of physical and emotional stress. Mobility and the ability to assist with personal care as needed. The moving and positioning of participants. Transportation of participants that requires loading/unloading of the vehicle. While performing the responsibilities of the Bridges, the employee is required to talk and hear. The employee is often required to sit and use their hands and fingers, to handle or feel. The employee is occasionally required to stand, walk, reach with arms and hands, climb or balance, and to stoop, kneel, crouch or crawl. Vision abilities required by this job include close vision.
