None SUMMARY:
Under the supervision of the Director and House Manager, this position is responsible for the care, supervision, and training of residents with disabilities residing in Momentum WORK, Inc. licensed homes.ESSENTIAL FUNCTIONS
Provides supervision and training to adults with developmental disabilities to promote growth in accordance with the person's individual support plan (ISP) Responsible for the implementation of each resident's program plan including data collection Reviews files for each resident on a regular basis Assists in the development and monitoring of individual and group programs Utilizes behavior modification techniques and other instructional strategies to enhance the progress of achieving goals and objectives Communicates concerns related to program participant needs to supervisor in a timely manner Acts as an appropriate role model Assists individuals in attending social activities to further social skills Always knows whereabouts of residents Responsible for daily routine(s) of residents and emergency medical procedures Maintains a dated, running log of observations to provide other staff with current information concerning residents Participates in cleaning the home, completing laundry, and preparing food as assigned Assists residents in personal hygiene and household routines Checks all outside doors consistently throughout the night by touring the home; ensures the security of the home Handles difficult behavioral situations with residents in danger of injuring themselves or others Acts as a liaison between other staff, supervisors, administrators, residents, and community members Attends meetings as assigned Completes records and reports as needed Assists with medication, records the centrally stored medication log, completes MAR and PRN forms, and notifies supervisor within 5 days of any medication in need of reorder Assists in maintaining weight logs, water intake logs, seizure logs, blood pressure logs, daily notes, behavioral documentation, and medical appointment checklists Complies with all Momentum WORK Inc., CCL & OSHA policies, procedures, and regulations Must be able to transport participants, unless waived by the Executive Director. Other related duties and responsibilities on an as needed basisPHYSICAL REQUIREMENTS
Ability to stand, sit for long periods of time Ability to go up and down stairs Ability to walk for long periods of time Ability to push or pull wheelchairs when necessary Ability to speak, read, hear, and understand technical written materials The employee is frequently required to climb, stoop, twist, or bend and lift up to fifty (50) pounds, using proper body mechanics to minimize injuries to self and people served Full awareness of environmental stimuli Ability to implement Crisis Management, Safety & Pro Act techniques (training included)Note:

: High school diploma or equivalent At least one (1) year of experience working with persons with developmental and/or other disabilities in individual and/or group settings preferred State of California DSP II certification Demonstrated ability to maintain the highest standards of confidentiality regarding persons served and other related information Excellent verbal and written communication skills Ability to complete tasks with numerous interruptions Compassion and respect for persons with disabilities Ability to demonstrate patience and understanding because people served may have intellectual, emotional, or behavioral challenges, in addition to physical limitations May need to be available during any disasters, per disaster plan Must have reliable phone Valid certification in CPR/First Aid (Training included)BENEFITS
