Job Description

Director of Admissions West Hempstead, NY $100,000 a year - Full-time $100,000 a year - Full-time A skilled nursing and rehabilitation facility in New York is seeking an experienced Director of Admissions to oversee the full admissions process and support census stability. This role is ideal for a strong admissions leader who understands hospital referrals, payer requirements, and how to manage smooth transitions into long-term and post-acute care.
Responsibilities:
Oversee all admissions from referral through admission Coordinate with hospitals, case managers, discharge planners, and referral sources Review clinical documentation to ensure appropriate placement Manage insurance verification, authorizations, and payer requirements Track referrals, conversion rates, and census trends Work closely with nursing, social services, and administration to ensure seamless transitions of care Serve as a point of contact for residents and families during the admissions process Maintain compliance with state, federal, and facility admissions policies
Qualifications:
Prior admissions leadership experience in a skilled nursing facility required Strong knowledge of Medicare, Medicaid, and managed care admissions Experience working with hospital discharge planning teams Excellent communication, organization, and follow-up skills Ability to manage high referral volume in a fast-paced environment
