Full Job Description Coastside Adult Day Health Center Job Title:
Director of Nursing Reports to: Program Director Coastside Adult Day Health Center operates a four hour daytime healthcare program in Half Moon Bay, CA 94019. We provide care and support for seniors and adults with disabilities throughout all of San Mateo County. This position requires a person who is licensed by the California Board of Registered Nursing with a background in public health or gerontology.
Duties:
Assess and implement nursing and personal care needs of each participant, including current drugs and evaluation of healthcare needs. Develop a plan of care for each participant based on initial and re-evaluation assessments. Supervise LVN in providing participant care, if applicable based on census. To teach self-care skills and assist participants to remain independent, and to provide health education to staff, families and caregivers. To record and maintain flow sheets and progress notes on all participants indicating nursing and personal care services provided as well as documentation required as per title XXII for TARS. To make provision for emergency care, including arrangements for transportation and communication with acute care service providers To confer with physicians, health professionals and community agencies on behalf of individual medical needs. To notify physicians on any marked change in participants' condition. To maintain community liaison with health professionals and other community medical resources. To coordinate participant nutrition and portion control in collaboration with the Dietitian. Administer medications and take vitals as documented in individualized plan of care. Follow up daily with participants who are expected in attendance but are considered a "no show". To perform other duties as assigned by the Program Director and/or the Executive Director.
Qualifications:
A good work record, clean, neat, reliable and capable of performing above duties and other duties assigned. Good people skills, including empathy and sensitivity to needs of elderly and disabled populations. Must collaborate and work well within the team.
Wage:
$45 per hour subject to negotiation based on experience.
Schedule:
