POSITION SUMMARY
The Nursing Supervisor is a Registered Nurse (RN) who is responsible for the overall supervision of nursing care at High Point Center for Care in both Assisted Living and Memory Care. He/she will ensure the safety and well-being of all residents; provide direct resident care activities to include assessments, planning, implementing, and evaluation of nursing care. He/she will provide managerial and/or clinical support to residents and staff within High Point Center for Care to promote good health and achieve high quality of life. He/she will supervise the care provided by Licensed Practical Nurses (LPN) and Quality of Life Specialists (HHA).ESSENTIAL JOB FUNCTIONS
Ensure nursing staff (LPN) and Quality of Life Specialists (HHA) provide quality, compassionate, and appropriate resident care. Provide daily supervision and leadership to assigned staff and other designated personnel in meeting the residents' needs. Demonstrate good decision-making skills. Provide written evaluation and oral counseling of assigned staff. Provide support to Quality-of-Life Specialists (HHA) and LPN staff. Responsible for maintaining charting and documentation on each resident's status. Provide nursing care according to physician's orders and in conformance with recognized nursing practice, established standards, and administrative policies. Responsible for completing vital signs (i.e. temperature, pulse, respiration, and blood pressure) as needed. Responsible for treatments as ordered by resident's physician and/or nurse practitioner (i.e. dressings, catheterization, suctioning, oxygen, etc.) Ensure physicians orders are carried out within a timely manner. Ensure Narcotic counts are accurate. Communicate with health care providers to ensure residents have all medication orders and supplies for their care. Communicate with resident family members as needed on all issues/concerns for residents. Responsible for scheduling medical appointments and arranging transportation for appointments as needed. Participate in the assessment and admission of new residents. Participate in discharge planning as needed. Respond to residents call system promptly and courteously. Provide emergency care/first aid within scope of practice as needed. Assist as needed with recruiting, hiring, providing orientation/training, and retaining nursing staff. Responsible for identifying resident medical and behavioral changes and developing and monitoring of residents' care plans. Provide daily report to the Director of Health Services concerning facility conditions, personnel performance, resident conditions, and needs for nursing supplies. Communicate with interdisciplinary team on pertinent information regarding resident conditions. Assess and monitor complex resident health status and determine the appropriate nursing intervention. Develop and monitor the ongoing plan of care with residents, their representative, Health Services Director, and interdisciplinary team and initiate corrective action as necessary. Make rounds with physicians and assist in examinations and/or treatment of residents. Assess residents and notify their physician(s) and responsible party in the event of a change in the resident's condition. Orient residents and staff to emergency procedures and respond to all emergency situations. Work with Health Services Director to maintain and monitor Department of Health regulatory requirements as it relates to nursing. Ensure that all residents' records are accurate, complete, and all mandatory forms are on file. Monitor resident environment to ensure all safety and sanitary regulations and quality standards are met or exceeded. May assist with development and presentation of in- service programs. Responsible for scheduling Quality-of-Life Specialists (HHAs) and LPNs to ensure continuous 24-hour coverage. Responsible for finding coverage for assigned staff when call outs occur. Covers shifts as needed when LPN is not available. Perform other duties as assigned.EDUCATION REQUIRED
Must be a graduate from an accredited school of nursing with a current New York State Registered Nurse (RN) license. CPR Certification is required. Experience with working with Alzheimer's and Dementia residents is preferred. Two (2) years of supervisory/management experience preferred. The ability to speak, read and comprehend the English language. Ability to learn and work with the specific needs of those residents suffering from Alzheimer's disease and/or other related dementias. Willingness to be available for any/all emergencies regarding the facility. Must possess the ability to make independent decisions when situations warrant such action. Familiarity with Microsoft Office Suite products and the ability to work with an electronic medical record system. Agreement to complete orientation program for Heartfelt Connections Program and other community-based training programs. Must display patience, tact, enthusiasm, and a cheerful disposition. Must be willing to learn new techniques and principles and willing to incorporate them into existing resident care practices. Must have knowledge of basic nursing care techniques. •Must possess excellent communication and customer service skills • Ability to work as a team and display a positive attitude.
