At Silver Creek, we believe that excellent care goes beyond clinical outcomes. It is about dignity, communication, advocacy, emotional support, and helping each resident and family feel heard, respected, and supported throughout their stay. The Director of Social Services plays a key role in the resident experience by supporting admissions, discharge planning, care coordination, family communication, psychosocial well-being, and regulatory compliance. Position Summary The Director of Social Services is responsible for planning, organizing, and directing the Social Services Department in accordance with federal, state, and facility guidelines. This role works closely with residents, families, nursing, therapy, admissions, case management, and the interdisciplinary team to ensure resident needs are identified, addressed, and documented appropriately. Key Responsibilities Complete psychosocial assessments, social service evaluations, progress notes, and care plan documentation timely and accurately. Participate in care plan meetings, discharge planning meetings, IDT meetings, and family conferences. Support residents and families through the admission, adjustment, care planning, and discharge process. Coordinate safe and appropriate discharge plans, including referrals to community resources, home care, equipment, transportation, and follow-up services. Advocate for resident rights, dignity, self-determination, and quality of life. Assist with grievance resolution and family concerns in a professional and timely manner. Collaborate with nursing, therapy, admissions, business office, and outside agencies to support continuity of care. Maintain compliance with all applicable federal and state regulations related to social services in a skilled nursing facility. Assist residents with advance directives, healthcare proxy information, financial/resource referrals, and other support needs as appropriate. Monitor residents for psychosocial changes, mood concerns, adjustment difficulties, behavioral needs, and emotional well-being. Support survey readiness through accurate documentation, timely follow-up, and resident-centered care planning. Qualifications Bachelor's degree in Social Work, Human Services, Psychology, Sociology, or related field preferred. Previous experience in a skilled nursing facility, long-term care, rehabilitation center, hospital, or healthcare setting strongly preferred. Knowledge of discharge planning, resident rights, care planning, and long-term care regulations preferred. Strong communication, documentation, organization, and problem-solving skills. Ability to work collaboratively with residents, families, staff, physicians, hospitals, and community partners. Compassionate, professional, and resident-centered approach. Ability to manage multiple priorities in a fast-paced healthcare environment.
