Job Description

Direct Service Professional Reports to Designated Coordinator Purpose Provide community integration and inclusion through new skill building, life enrichment activities and recreational/leisure activities. Provide training and support services including positive approaches to learning, systematic instruction, behavioral intervention, and self-care needs. Provides transportation services for individuals. Provide 1:1 coaching and support for those clients participating in community employment. Key Expectations 1.   • Learn and consistently perform person-centered practices: Identify those things that are important to people who receive support; implement strategies to assure they are present in day-to-day life.
Identify areas where change can happen without permission. Act on those areas. Challenge co-workers and supervisors to use person-centered practices. 3.
Safety:
Participate in the development of a safe and healthy workplace. Comply with safety policies and procedures and adhere to safe work practices. Comply with state motor vehicle laws and regulations. Take reasonable care to ensure your own, co-workers, and Individuals served safety and health. Report any hazards, injuries, or illnesses immediately to your supervisor. Will not place others at risk by any act or omission or interfere with safety equipment. Act as a role model by demonstrating safe work behaviors. Essential Functions 1. Direct Support Care
  • Provide direct support for the individuals in this program using a person-centered approach including lifting and transferring adults to/from wheelchairs.
Carry out self-care and support services (toileting, feeding, personal hygiene needs) as designated by the daily schedule. Assist other staff with strapping wheelchairs, transferring of individuals from the vehicles. Provide ongoing support for behavioral concerns, provide de-escalation and intervention as needed. Provide therapeutic programming as recommended by OT, PT, and speech therapies. 2. Dispense Medication
  • Certified to pass medications and perform specific medicinal treatments/cares according to individual need. 3. Support the daily operations and needs of the program. Assist in general cleaning and caretaking of the building (loading dishwasher, doing laundry, picking up work materials at the end of the day, cleaning and maintaining the refrigerator, etc.). 4. Communication
  • Speak clearly; listen to others; write effectively; establish relationships; act with integrity; establish trust; demonstrate a high level of respect and patience with Individuals served and coworkers.
Communicates with other program staff and residential staff on a daily, or as needed basis, regarding program issues, caseload issues, staffing, and other related information. 5. Crisis Prevention and Intervention
  • Be knowledgeable about crisis prevention, intervention, and resolution techniques and match such techniques to circumstances and individuals.
Identify the crisis, defuse the situation, evaluate, and determine an intervention strategy and contacts necessary supports. Continue to monitor crisis situations, discussing the incident with authorized staff and person(s) served, adjusting supports and the environment, and complying with regulations for reporting. Use appropriate first aid/safety procedures when responding to emergencies. 6. Lead with a service focus
  • Set the example for and lead others in focusing on providing excellent service for people served, their families, and all other people who support them; etc.
7. Provide transportation services as designated by the driving schedule, drive and/or ride along on route, as needed or assigned. 8. Complete all required annual trainings on time using Hope DAC's online learning management system or alternate delivery method if offered. 9. Ensure that all required documentation and paperwork is completed accurately and according to current statute and regulations. 10. Represent Hope DAC and the programs/services offered within the local and surrounding community in a manner that promotes the organization. 11. Complies with Hope DAC policies and procedures. 12. Provide 1:1 employment support by coaching, demonstration, modeling, etc. when supporting individuals in the community employment setting, Provide proper examples of good employee behavior to individuals served. Develop, facilitate, attend, and/or participate in community meetings/events, in-service trainings, workshops, and/or classes as apparent or directed. 3. Keep your Supervisor informed of important developments, potential problems, and related information necessary for effective program management. 4.   • Willingness to learn online training system, and future software possibilities for documentation, time-keeping, and company email.
2. Familiarity or willingness to learn systems of adaptive communication and assistive technology in areas of software, hardware, and mobility. 3. Current, valid MN driver's license 4. A driving record that demonstrates safe, legal driving habits Ability to pass criminal/DHS background check.
  • CPR and 1st Aid Certification (we will provide if needed.
