About Us:
Consumer Support Services, Inc. is trusted in communities throughout Ohio as a provider of housing and support services to people with disabilities. We have a long history of empowering individuals, by working in partnership with their families or guardians to be responsive to their loved one's specific needs. We are looking for highly motivated individuals for the position ofTEAM LEAD
($17.50 hourly) in the Greenville area to assist individuals with developmental disabilities, focusing on quality care and daily activities.Responsibilities:
Provide direct services to the consumers based upon their Ohio ISP. Complete necessary documentation in individual's records as is necessary per CSS policy and procedure, licensure, Medicaid standards, DODD and any other governing agencies that are applicable to ensure that proper care is of the individual taking place. Provide input to Site Supervisor/Site Manager concerning the individual's needs. Ensure that the individual's rights are observed. Complete on-call services as assigned. This includes assisting with covering any open shifts or medical appointments at their assigned sites if they cannot be filled with DSP staff. This task also includes carrying the company assigned cell phone as assigned per county. Attend all staff meetings and in-services that are assigned. Communicate to your Site Supervisor/Site Manager any behavioral, medical or medication problems that should arise. Communicate to your Site Supervisor/Site Manager about any incidents of unknown origin as well as any UIR/MUI as assigned. Communicate any staffing issues with Site Supervisor/Site Manager. Assist in New Employee Training as assigned. Oversee the living arrangements of individuals. Ensure compliance with consumer's Bill of Rights. Manage individual accounts per their interdisciplinary plan, ensuring accountability for all individual funds. Assist with medical appointments as needed. Prepare assessments and reports concerning individuals as requested. Ensure that all needs of the individuals assigned are fulfilled. Ensure that the home meets all regulatory standards set forth by governing agencies. Adhere to the budgetary guidelines as outlined by your Site Supervisor/Site Manager. Complete all duties as assigned in the DSP Job Description. Communicate with all staff at the assigned site with any issues that may arise. Adhere to all CSS Policy and Procedure. Serve as an advocate according to CSS Policy and Procedure. Adhere to the NADSP Code of Ethics. Adhere to all safety standards set forth per regulatory agencies and CSS. Perform duties all other duties as assigned by the employer.Requirements:
High school diploma/GED required Proof of auto insurance required Valid driver's license required Able to pass background requirements & Pre-employment drug testing SS card OR Birth Certificate Proof of bank account for direct deposit- What we offer you:
