Job Description
DUI Care Coordinator
- Northeastern Kansas Region Topeka, KS 66617 From $25 an hour

- Full-time DUI Care Coordinator / Substance Abuse Specialist
- Northeastern Region Conduct comprehensive assessment, referral and ongoing monitoring of the clinical substance abuse needs of clients following sentencing guidelines of DUI offenders with 3 or more convictions, in order to reduce recidivism, enhance risk reduction and public safety.
DUTIES & RESPONSIBILITIES
Participate in the referral and screening process to determine program eligibility. Coordinate client care through integrated service planning to address multiple aspects of a client's life; collaborating with the client and key community partners to establish a framework of action to assist client in achieving specific goals/tasks while also collaborating with community resources. Connect and advocate for clients in obtaining access to services within their local and regional community to address personal and systemic barriers, this includes: o Assisting with obtaining safe, sober housing through transitional or permanent housing o Accessing mental health services to include medication management. o Connecting to substance abuse services such as: assessment for diagnosis and formalized treatment programs, community support groups, medication assisted treatment, recovery-oriented systems of care (ROSC) and/or recovery coach / peer mentors. o Addressing physical health issues that result from chronic substance use, such as high blood pressure, diabetes, dental hygiene, and/or traumatic brain injury, etc. o Assisting with seeking earned income, and/or the application for state and federal benefits, when appropriate and/or employment opportunities. o Support in navigating legal system and legal requirements for justice involved clients. Participate as a key member of interdisciplinary teams; facilitate early intervention and reduction of barriers to increase the client's opportunity for success and self-sufficiency. Function as an integral part of a behavioral health care system through the administration of appropriate diagnostic tools, to include: SUD assessments, DSM criteria, and American Society of Addiction Medicine (ASAM) Patient Placement Criteria, to determine diagnosis and recommendation of appropriate placement at the safest and most appropriate level of care to meet the client's individual needs. Provide accurate and timely documentation of client / system's contact for outcome and productivity purposes. Monitor and report on achievement of outcome goals by documenting client's involvement and progress of data outcome measures; developing a working recovery plan of goals/tasks identified by client. EDUCATION & EXPERIENCE
Masters or Bachelor level preparation with experience in social work, health care related field, and behavioral health or substance abuse systems is preferred. Licensure through the Behavioral Sciences Regulatory Board (BSRB) of KS is preferred: LAC- Licensed Addictions Counselor LMAC

- Licensed Marriage & Family Therapist LMLP
- Licensed Masters Level Psychologist LMSW
- Licensed Masters Social Worker LPC
- Licensed Professional Counselor Consideration given to candidates with extensive experience working with diverse communities around substance misuse and/or behavioral health.
