What You'll Be Doing:
Build relationships with patients, families, providers, and community stakeholders to improve care and health outcomes Assess and prioritize patient/community health education needs to support preventive care Provide individual and group education on chronic disease management, HIV/AIDS, asthma, cardiac health, diabetes, family planning, obesity, pregnancy, smoking cessation, and prevention Conduct assessments, orientations, and enrollment support for programs such as CPSP, Family PACT, and Every Woman Counts Coordinate post-hospital discharge follow-up, including scheduling appointments and ensuring continuity of care Support high-risk patients with self-management education, referring to RNs when clinical intervention is needed Serve as patient advocate, collaborating with providers, nurses, social workers, and medical assistants Participate in quality improvement initiatives (chart audits, data collection, performance activities) Provide age-appropriate education across the lifespan (infants through geriatric patients) Maintain knowledge of mandatory reporting requirements (domestic violence, child/dependent adult abuse) Maintain and coordinate health education materials, including audio-visual resources Document program activities per Omni Family Health and regulatory requirements Participate in community outreach, health fairs, and coalition meetings Perform additional duties as assigned
Qualifications:
Associate's Degree with 2+ years of experience in similar or community outreach/counseling work (Bachelor's preferred) Current BLS certification required; CPSP certification within one year of employment Willingness to obtain Health Education and/or Family Planning certification Bilingual (English/Spanish) communication skills, verbal and written Strong organizational, presentation, and interpersonal skills Ability to work independently and within a team, with sensitivity to diverse populations Willingness to travel to different locations and work weekends as needed
