Serving multiple counties, we strive to provide accessible resources and compassionate care to individuals and families across the community. Job Summary We are seeking a motivated and compassionate Drug and Alcohol Assessment & Case Management Specialist to join our dynamic team in Doylestown, PA. In this role, you will conduct comprehensive assessments using the ASAM Criteria, develop individualized service plans, and provide case management services to clients struggling with substance use and behavioral health challenges. Your expertise will help guide clients toward recovery and improved well-being through evidence-based practices and empathetic support. Responsibilities Conduct thorough clinical assessments including diagnostic evaluations using ICD coding (ICD-10) to determine client needs and treatment planning. Develop personalized case management plans focused on substance use treatment and recovery Document all patient interactions accurately in electronic health records while adhering to HIPAA regulations to ensure confidentiality. Coordinate with healthcare providers, community agencies, and families for integrated care delivery, including home & community care experiences when appropriate. Utilize research-based approaches to inform assessment and treatment strategies and improve patient outcomes. Requirements Minimum of a bachelor's degree in human services or related field. Must hold an LCSW or LPC license or be credentialed as a Certified Allied Addiction Professional (CAAP) or Certified Alcohol and Drug Counselor (CADC) through the Pennsylvania Certification Board (PCB) at the time of hire or be actively working toward PCB licensure or certification. Proven experience in assessment, substance use counseling, and case management. Thorough understanding of clinical terminology, documentation standards, DSM-5 diagnosis criteria , and HIPAA compliance. Skilled in assessment techniques including evaluation for mental health disorders and substance use disorders. Proficiency in applying therapeutic modalities such as Cognitive-Behavioral Therapy (CBT) and Motivational Interviewing (MI) and determining Stage of Change. Strong communication and rapport‑building skills to effectively engage clients. Join us in making a meaningful difference by empowering individuals on their journey toward recovery!
Pay:
$23.00 - $24.00 per hour
