293425128 Job Description The Assessment Specialist has the primary responsibility to assess and verify the basic program eligibility for individuals and families in need of direct services available through CAC, provide customer education, client information, referral and support services to individuals and families experiencing crisis or barriers to self-reliance; maintain detailed case records; interpret and clarify program specific policies and procedures; interview, screen, and process to determine an applicant's eligibility to participate in programs administered by the assigned department. 1. Conduct basic information orientations for applicants.- Provide program orientation and pre-application screening when required.
- Outline documentation requirements.
- Schedule eligibility appointments.
- Notify applicants of their rights, responsibilities, and grievance procedures. 2. Conduct eligibility interviews/determination.
- Interview applicants to obtain sufficient eligibility information.
- Gather documentation and other information required by program rules and regulations for eligibility.
- Determine and document eligibility as established by program guidelines.
- Determine levels of payment to be provided to vendors based on policies and procedures.
- Notify applicant of determination and provide services as required. 3. Data collection and file maintenance.
- Maintain eligibility information and documentation in files.
- Maintain records and reports.
- Maintain and update the program procedures manual.
- Prepare and maintain daily logs when required.
- Generate monthly performance reports as required. 4. Provide outreach and technical assistance to applicants and/or the community.
- Assist in the development and implementation of recruitment processes using various marketing strategies.
- Interact with other agencies to distribute and garner information when required.
- Build and maintain relationships with external social service providers to facilitate effective referrals and direct services. 5. Provide services.
- Review and process claims for payment.
- Issue vouchers.
- Coordinate required training.
- Coordinate data gathering and input.
