Open Until Filled Woodbridge School District is accepting applications for a Special Education Transition Coordinator. This is a full-time, 10-month, benefit-eligible specialist position.JOB SUMMARY
The Special Education Transition Coordinator supports students with disabilities in successfully transitioning from high school to post-secondary education, employment, and independent living. This role ensures that transition planning is meaningful, compliant, and aligned to each student's strengths, interests, and goals. The coordinator collaborates with students, families, school staff, and community agencies to develop and implement effective transition services as part of the IEP process.QUALIFICATION REQUIREMENTS
Must hold or be eligible for an educator license in the State of Delaware Must hold or be eligible for special education certification Master's degree in Special Education or a related field preferred Minimum of four (4) years of successful experience in special education preferred Knowledge of transition planning requirements under IDEA and IEP development Experience working with high school students with diverse learning needs Strong understanding of post-secondary pathways, including college, career, and vocational training options Ability to collaborate effectively with students, families, staff, and outside agencies Strong organizational, communication, and case management skills Experience coordinating services or programs preferredESSENTIAL DUTIES AND RESPONSIBILITIES
Coordinate and support the development of transition plans within IEPs for high school students (typically ages 14â€"21) Ensure compliance with federal, state, and district requirements related to transition services Collaborate with special education teachers, school counselors, and administrators to align transition goals with student needs Facilitate student-centered transition planning, including post-secondary education, employment, and independent living goals Connect students and families with community agencies, vocational programs, and post-secondary resources Organize and support job shadowing, internships, work-based learning, and career exploration opportunities Monitor student progress toward transition goals and adjust plans as needed Maintain accurate documentation and records related to transition services and outcomes Provide training and guidance to staff on best practices in transition planning Participate in IEP meetings and multidisciplinary team meetings Support students in developing self-advocacy, independent living, and employability skills Assist with data collection and reporting related to post-school outcomes and compliance indicators Stay current on laws, regulations, and best practices related to transition services Perform other duties as assigned Employment is contingent upon successful completion of all required background checks and clearances. This is a 10-month position based on a 188-day annual contract. Employees are expected to work 7.5 hours per day, consistent with the established school and district schedules. Responsibilities may include duties beyond instructional time, such as meetings, professional development, and school-related events. Employees are expected to maintain regular attendance, fulfill all assigned responsibilities, and demonstrate flexibility to meet the evolving needs of students, staff, and the district. This position may require additional workdays during the summer months beyond the standard school-year schedule.ADA ACCOMMODATIONS DISCLOSURE
