JOB SUMMARY
The Referral Management Coordinator is responsible for assisting skilled nursing facilities with census growth opportunities. The position will work with assigned facilities with meeting and exceeding their census goals and direct the admissions process from referring hospitals to the assigned facilities. Job responsibilities may include but are not limited to: Oversee referral and inquiry process for patient care services. Adequately triage incoming hospital referrals. Follow admissions process according to company procedures. Communicate admissions decisions timely to referral source. Analyze data trends and prepare reports as requested. Create positive resident and staff outcomes. Respect others by treating them the way we want to be treated. Demonstrate kindness, compassion, honesty, and integrity at all times. REQUIREMENTS
Bachelor's Degree or minimum 2 years' experience in healthcare, community relations, admission, sales or marketing Willingness to drive throughout the day Successful criminal background check Healthcare marketing, sales and admissions experience preferred.
