Admission Director Duties & Responsibilities:

Maintains a current file of all community and social service agencies whose services may be needed by any of the residents. Utilizes these services as needed for the residents by direct referral. Communicates with other department heads regarding needs of the residents which the admissions coordinator has known and which are the responsibilities of other departments. Serves as a liaison with the Nursing Department regarding admissions and patient relations. Facilitates a 15-minute acceptance turnaround time for all referrals/admissions with nursing. Provides pertinent information such as insurance cards, social security numbers, Medicare and Medicaid numbers to the necessary departments. Attends in-services and workshops as appropriate with approval of the Administrator. Responsible for placing and receiving calls from hospitals and families, obtaining necessary information from families and hospitals in regard to admitting procedures in conjunction with the Administrator. Keeps the Administrator and the Director of Nurses updated as to availability and suitability of new admissions. Responsible for completing various necessary forms. As an example, the Admissions Director may complete forms from the Social Security Administration. This will be coordinated with the Business office to determine which forms need social service attention.

Responsible for admissions screening:

screen faxes from hospitals regarding potential admissions. Screen incoming phone calls regarding potential admissions. Schedule tours of the facility and accommodate walk-in tours.

Responsible for admission tours:

obtain initial contact information e.g. potential admission's financial and medical status. Provide information regarding advanced directives, payment alternatives and passport services. Ensure that each admission has PAS/ID and

as appropriate, and obtain PAS/ID and

approval from discharge planners or from ODHS as appropriate.

Set up time and room for new admits, send out notices of admissions to department heads and nursing personal. Attend pre-admission conference, greet resident/family upon admissions and participate in sign-in of resident as appropriate. At this time, interview resident/family/responsible party to obtain information for the Social History. Prepare and type the social history and place it in the resident's medical chart. Responsible for rooms changes in the facility. Meet with appropriate staff members and notify or clarify room change. Meet with resident an/or legal representative/family member and complete and issue room change form. May be assigned other duties by job description, administrator, or regional support team.
