Admissions Coordinator -
Full-Time Troy Rehabilitation and Healthcare Center Location:
Troy, Ohio Troy Rehabilitation and Healthcare Center is seeking a positive, energetic, and highly self-motivated Admissions Coordinator to join our team full-time. This position plays an essential role in welcoming new residents and ensuring a smooth, timely, and supportive admissions experience for residents and their families. The ideal candidate thrives in a fast-paced, constantly changing environment, takes initiative, and remains professional and solutions-focused under pressure. Previous experience performing admissions in a long-term care setting is required. Key Responsibilities Coordinate the admissions process from initial referral through resident arrival. Review referrals and clinical documentation for completeness. Communicate promptly and professionally with prospective residents, families, hospitals, referral sources, and internal team members. Obtain and verify all required admission documents, authorizations, and financial information. Collaborate with nursing, social services, business office, therapy, and other departments to prepare for each admission. Maintain accurate records and update admissions systems, referral platforms, and census information. Provide tours and information to prospective residents and their families. Follow up on referrals and respond quickly to inquiries. Help ensure new residents and families receive a warm, positive welcome. Maintain confidentiality and comply with all applicable facility policies and regulatory requirements. Assist with additional admissions and census-development responsibilities as needed. Required Qualifications Previous admissions experience in a long-term care or skilled nursing facility is required. Strong organizational skills and exceptional attention to detail. Positive, professional, and welcoming attitude. Self-motivated with the ability to take initiative and work independently. Ability to prioritize multiple responsibilities and meet deadlines. Comfortable working in a fast-paced, frequently changing environment. Excellent written, verbal, and interpersonal communication skills. Strong problem-solving and follow-through abilities. Ability to work collaboratively with residents, families, referral partners, and facility staff. Proficiency with computers, electronic records, and standard office applications.
