The Transitional Care Liaison conducts and supervises the long-term care admissions process in order to achieve and exceed occupancy goals for the life plan community in accordance with company standards and federal, state, and local standards, guidelines, and regulations. Essential Activities and Tasks Administration and Reporting
- 50% Exercises discretion and independent judgment with respect to matters of significance in relation to admissions.
- 20% Grows the referral source pool and manages referral sources including informing them of services offered, strengths, accomplishments, and availability of the life plan community.
- 10% Provides the Corporate Finance Department with pre-certifications and other documentation for billing purposes.
- 10% Fosters, cultivates, and maintains positive relationships with the residents and their families. Provides timely and clear information to prospective residents, patients, family members, staff members, and third party payers. People Management and Development
- 10% Manages assigned staff at the life plan community.
- Up to 6 hours/day Standing
- Up to 6 hours/day Walking
- Up to 6 hours/day Lifting, pushing, pulling, and moving equipment, supplies, etc.
- Up to 25 pounds Driving
- Up to 6 hours/day Work weekends, evenings, and holidays
- Occasional On-call availability
- 24/7 for admissions Subject to residents/patients with various disease processes
- Minimal May be exposed to infectious waste, disease, conditions, etc. including exposure to the AIDS and Hepatitis B viruses
- Work day Risk Category for Exposure to Bloodborne Diseases
