SUMMARY OF JOB RESPONSIBILITES
This is an internal and external customer service position responsible for coordinating the referral-to-admission process for hospice patients. This individual provides administrative support to the admissions team to ensure that all eligible patients receive care and assures all barriers to admission are addressed and resolved leading to agency growth. This position requires communication with physicians, discharge planners, care coordinators, case managers, social workers, as well as the general community. The Community Relations Coordinator reports to the Executive Director of Business Development Responsibilities and Duties include but are not limited to: Ensures timely transition of referrals into admissions identifying barriers and solving customer service and other issues through partnership with the sales, clinical and operations teams. Spends time engaged in coordination of referral-to-admission process from initial inquiry, patient data entry, and scheduling of admission. Maintains thorough knowledge of Cardinal Hospice Care services, acuity capabilities and referral relationships. Responds to website inquiries on Cardinal Hospice Care services. Gathers and records data information during the intake process. Utilizes electronic medical record systems from referral sources to obtain patient information when given access. Provides information/tracks statistics and trends for quality care and QAPI reporting. Communicates and educates referral sources about services via written and telephone contacts. Visits referral sources to provide agency recognition and top customer service. Investigates, verifies, and documents coverage from all payer sources. Provides daily information on census, referrals, admissions, referrals pending, non-admits, discharges, and issues/concerns relating to the intake process to leadership. Maintains an educated understanding of all payer sources and impact on revenue. Ensures that admission packets are available for admission team members. Assists in making follow-up calls to referral sources and patients/families. Learns and executes given selling techniques to lead to appropriate patient, family, or referral source commitment. Be available to participate in growth meeting including reporting of progress of actual vs. budgeted SOCs and census. Coordinates scheduling of staff for onsite information gathering, patient evaluations, patient consents, and admissions. Provides excellent internal and external customer service and promotes appreciation of customers with other staff. Maintains referral/admission, discharge, follow-up tracking boards. Acts as a brand champion understanding and articulating Cardinal Hospice Care differentiators. Performs any miscellaneous assignments as may be required or perform any tasks as assigned by supervisor.

PHYSICAL DEMANDS
Lifting 25 pounds maximum with occasional carrying, pushing, pulling, or otherwise moving objects.
