A Middle or High School Teacher teaches middle school students, usually in grades 6 - 12, in a public or private school. Teaches one subject, or a group of subjects, such as math and science; works with one class all year or teaches different classes of students throughout the day. May mentor students, coach a school team or advise a student club. May specialize in working with students learning English or students with learning disabilities or emotional and behavioral disorders.

Click on "Search for an Educator," then use the secured search option by entering your Social Security number and birthdate. All documents and credentials associated with your record will be listed. Select the appropriate document number, right-click, then select "Print." Change the destination to "Save as PDF," and save the file for submission.
