We are recruiting for a Full-Time Medical Day Care Social Worker to join our skilled team of pediatric professionals in Northfield, NJ. This position is benefit eligible.

POSITION SUMMARY
The Medical Day Care Social Worker (SW) provides comprehensive psychosocial assessment, care coordination, and family support services to pediatric patients and their families. The Social Worker is a key member of the Interdisciplinary Team (IDT) and is responsible for: Conducting psychosocial assessments Developing and contributing to individualized care plans Facilitating family involvement in care Providing education regarding disability and care needs Coordinating community resources Providing counseling, home visits, and discharge planning The SW supports the emotional, social, and financial well-being of children and families while ensuring compliance with: Facility policies and procedures NJ Department of Health Pediatric Medical Day Care standards Interdisciplinary care planning requirements Confidentiality and regulatory standards
POSITION QUALIFICATIONS
Licensed or certified by the New Jersey Board of Social Work Examiners Minimum one (1) year experience in pediatric or family social work Strong communication and interdisciplinary collaboration skills Computer proficiency (EMR, Microsoft Office, documentation systems) Knowledge of community resources and support systems CPR Certification (BLS)
