Casa de la Familia specializes in the treatment of post-traumatic, anxiety, and depressive disorders through crisis intervention and long-term individual and family therapy. We collaborate with city, county, and state agencies, healthcare providers, community organizations, victim advocacy programs, and social service partners to support underserved and underrepresented populations through culturally responsive services. Position Summary The ECM Care Coordination Specialist supports Casa de la Familia's Enhanced Care Management (ECM) program through CalOptima. This position serves as a primary point of contact for members and assists with outreach, engagement, appointment coordination, resource linkage, and documentation activities. The ECM Care Coordination Specialist works closely with Care Managers, healthcare providers, and community organizations to help members access medical, behavioral health, and social support services. The ideal candidate possesses strong communication skills, enjoys working with diverse populations, and demonstrates a commitment to improving access to care for individuals with complex needs.
