Assesses students and helps to develop individualized education plans that specify the services and accommodations needed for each student. Works in a classroom that includes only students with IEPs, or works with students and teachers in regular classes. For students with severe disabilities, may focus on developing skills for independent living. Works as part of a team that includes therapists, school counselors or social workers.

Key Responsibilities Provide one-to-one or couple care in clients' homes with compassion and professionalism Assist clients with daily living activities including personal hygiene, mobility, and medication reminders Support clients with special needs and children's care in the Stockton/Manteca/Tracy area Maintain accurate records and communicate effectively with clients and team members Qualifications & Requirements At least 1 year of prior caregiver experience, preferably with acute, skilled nursing, or residential care
CNA/CHHA
certificates are a plus; Home Care Aide Registry registration preferred (assistance provided if needed) Eligible to work in the United States and 18 years or older Ability to lift 50 lbs. and push 300 lbs.
