SUMMARY The Behavioral Health Specialist is a member of the Center for Population Health's Enterprise Ambulatory Care Management team and is accountable for improving the health outcomes of the populations being managed. Primary Care, Ambulatory Care Management teams, and healthcare professionals across the Yale New Haven Health System (YNHHS). This role is responsible for the development, implementation, coordination, and evaluation of behavioral health services for patients across the continuum of care. The Behavioral Health Specialist is a licensed, experienced clinician who serves as a subject matter expert in the assessment, treatment, and coordination of behavioral health services across YNHHS delivery networks.

RESPONSIBILITIES
The behavioral health specialist reports to the Manager and works in collaboration with physicians, Care Coordination staff, internal resources, Patient Navigators, pharmacists, and other healthcare professionals to develop, implement, evaluate and revise the delivery of behavioral health services to the employee/patient population. Conducts diagnostic assessments and providing brief, evidence-based behavioral health interventions. ' Supports YNHHS Primary Care practices with behavioral health integration. Consults with primary care providers, care managers, and multidisciplinary teams. Provides clinical supervision, staff support, and professional development through in-services and consultation. Enhances clinical practice standards and supporting program development initiatives. Collaborates closely with YNHH Psychiatry and community-based behavioral health providers. Supports activities to align with organizational goals, including readmission and risk reduction, improved access to care, and system -wide behavioral health integration. Partners with providers, care coordinators, vendors, and community agencies to ensure consistent, high-quality, and coordinated behavioral health care delivery. Leads a behavioral health care management team. Streamlines behavioral health referral processes and care coordination Delivers behavioral health education and consultation to Enterprise Ambulatory Care Management teams.
Qualifications:
EDUCATION
Masters degree in Social Work or a related behavioral health field from an accredited institution.
EXPERIENCE
Minimum of five (5) years of clinical experience addressing behavioral health needs across the spectrum of care
SPECIAL SKILLS
Commitment to a patient-centered philosophy of care Demonstrated cultural competence with sensitivity to diverse populations and beliefs Strong interpersonal, communication, and leadership abilities Ability to function effectively as a multidisciplinary team leader and behavioral health expert
LICENSURE/CERTIFICATION
LCSW and or related Behavioral Health License to practice in
CT PHYSICAL DEMAND MMR
is required. Varivax (chicken pox vaccine) or evidence of prior chicken pox is required. Hepatitis B (or signed declination) is required for those with potential exposure to blood/body fluids. Tdap strongly encouraged and influenza vaccination required. Must be able to travel to various delivery network locations.
