$38 per hour
Position Description:
We are seeking a Behavioral Health Case Manager to support discharge care coordination, transition of care, and episodic case management for acute conditions. This role focuses on post-discharge follow-up, member engagement, and coordinating care between providers, facilities, and internal teams to improve health outcomes and reduce readmissions. You will work independently while collaborating with physicians, discharge planners, and care teams to ensure members receive appropriate resources, education, and support throughout the care continuum.
What You'll Do:
Discharge care coordination and transition of care support. Conduct post-discharge follow-up and member outreach. Collaborate with providers, physicians, and care teams to coordinate services. Provide patient education, counseling, and resource referrals. Assist members with appointment scheduling and care navigation. Identify and address barriers to care and treatment adherence. Support medication reconciliation and adherence education. Refer members to internal programs or external community resources. Assist with pre-admission/post-discharge planning and alternate treatment options. Evaluate case complexity and escalate to appropriate departments when needed. Promote behavior change and self-management strategies. Assess quality of care concerns and escalate to QA/QI when appropriate. Maintain compliance with HIPAA and regulatory standards. Participate in onboarding and training of new team members as needed.

Licensed
LCPC, LCSW
(preferred) or RN with behavioral health experience. 2+ years of clinical experience. 1+ year of managed care or health insurance experience. Experience with discharge planning, case management, or care coordination.
Even Better, You May Have:
Previous experience working for a health plan or payer. Experience with high-risk or complex member populations. Behavioral health-focused case management experience. Familiarity with care management systems and databases.

Payer Experience Behavioral Health Case Management Education Required:
Associate's or Bachelor's degree in relevant field Active clinical license (LCPC, LCSW, or RN)
Education Preferred:
