Reports to: This position reports to a CASE Collaborative Program Administrator
Job Goals:
To teach a transition program for students aged 18-22 on track to receive a high school diploma. To implement educational and transition services for students in this program. To work collaboratively with parents/guardians and colleagues to meet the academic and transitional needs of students with disabilities.
Performance Responsibilities:
Program Development:
Create and maintain a transition curriculum and post-secondary learning opportunities; develop local employment opportunities and coordinate the school's community work program for students as needed.
Curriculum Alignment:
Deliver instruction in core content areas (English, Math, Science, Social Studies) aligned with State Curriculum Frameworks and Common Core Standards; select appropriate methodologies for transition instruction.
Differentiated Instruction:
Implement high-quality instruction in classroom and community settings, utilizing behavioral management techniques, support plans, and modifications/accommodations.
Service Delivery:
Develop and implement student schedules in accordance with IEP service delivery; coordinate with specialists as needed.
Evaluations:
Administer, analyze, and write reports for educational and transition assessments (including three-year reevaluations) to inform the IEP process.
IEP Development:
Draft data-driven IEP goals focused on both academic mastery and post-secondary outcomes. Collaborate with Teams to write and update all aspects of the IEP, including service delivery grids and Transition Planning Forms.
TPF & SOP
Development:
Develop a comprehensive Transition Planning Form (TPF) and the Summary of Performance (SOP), documenting the academic achievement and functional performance for students aged 18-22 and identifying specific needs, activities, and services to meet post-secondary goals.
Progress Monitoring:
Maintain rigorous data on student progress toward IEP goals, complete data-informed progress reports and report cards according to Collaborative and regulatory schedules, and provide such reports to administrators.
Meetings:
Participate actively in annual reviews, reevaluations, and Open Houses; lead parent-teacher conferences and present assessment data to the Team. Participate in all required Collaborative meetings.
Behavioral Support:
Collaborate with BCBAs to implement behavioral interventions and support plans for students with social-emotional needs.
Credit Recovery:
Monitor student progress toward graduation requirements; coordinate with guidance departments to ensure all "units of study" are completed for diploma eligibility.
College Readiness:
Assist students in navigating the college application process, practicing self-advocacy, and preparing for entrance exams as needed.
Financial Literacy:
Provide instruction related to the effective use of various financial skills, including budgeting, investing, saving, and managing debt.
Vocational Experiences:
Cultivate and monitor work-based learning opportunities and community internships.
Self-Advocacy Coaching:
Explicitly teach students to understand their own IEPs, request accommodations, and manage their disability in adult settings.
Executive Functioning:
Provide direct instruction to students in organization, time management, and self-regulation skills necessary for the workplace or a college campus.
Internet Safety:
Teach students skills to protect their personal information, devices, and well-being from digital risks while using the internet.
Stakeholder Liaison:
Maintain frequent communication with parents/guardians and consult with therapists and counselors to support students' developmental and social needs.
Crisis Management:
Report student/family concerns or crises to Program Administrators immediately and maintain strict confidentiality and professionalism.
Staff Leadership:
Supervise, schedule, and provide evaluative feedback for teaching assistants; provide coaching and modeling to ensure instructional consistency.
Resource Management:
Order necessary instructional materials and maintain technology/adaptive equipment.
Agency Collaboration:
Coordinate with adult service agencies (e.g. Massachusetts Rehabilitation Commission) to ensure a smooth transition upon graduation.
Professional Growth:
Engage in professional development to maintain licensure and stay current on research and best practices in special education and transition.
Professional Responsibilities:
Annually complete the MA Educator Evaluation process based on the educator's role and responsibilities as well as his/her professional practice and student learning needs.
General Duties:
Perform other related tasks at the discretion of the Program Administrator or Executive Director, including supervision of students during non-academic periods. This position falls within the Agreement between The CASE Professional Association/MTA/NEA and The CASE Collaborative (Unit A).
