Join Our Team at Avita Health System Avita Health System Multiple LocationsAvita Health System is proud to serve the communities of Crawford and Richland counties through three hospitals and numerous clinic locations. Over the past few years, weve tripled in size, now employing over 2,300 team members and more than 200 physicians and advanced practitioners. Our mission is to deliver high-quality, compassionate care to the people who depend on us.

Were currently seeking a dedicated Diabetic Care & Education Specialist Nurse to join our Diabetic Education Department at our hospital locations.

Position OverviewPerforms outpatient professional nursing consultation and educational services for persons with diabetes of all types and ages in the form of individual appointments, classes, and community events. Assists in the development of curriculum, including the most up-to-date research. Education to be provided includes the necessary lifestyle changes involved with a diabetes diagnosis, which includes but is not limited to; activity, nutrition, social barriers, medications, costs, physical activity, glucose testing, problem-solving, and financial/food assistance. Assists with developing educational policies and procedures. Also responsible for the direction, implementation, and evaluation of the Diabetes Self-management education (DSME) program and its objectives.
QualificationsRequired:
Current State of Ohio Registered Nurse LicensureAssociate Degree in Nursing Current Certified Diabetes Care & Education Specialist (CDCES) Certification BLS CertificationMinimum of 2 years experience as a
Certified Diabetes Care & Education SpecialistPreferred:
