Key Responsibilities Creates and maintains consumer charts including filing, thinning, closing, etc. Completes ROI packets as requested. Assists with the processing of PCP notifications, medical record requests, and refill requests. Supports the Morning Meeting and weekly Integrated Care Meeting protocols through management of the meeting logs and daily assignment schedules. Manages the team referral status roster, including collection of baseline data. Coordinates and monitors scheduling for the assigned team. Responsible for maintaining Prior Authorizations for services and assists medical staff with medication Prior Authorizations. Acts as a liaison between team and consumer (greets, receives, and escorts consumer to appointment when needed). Qualifications Must be a minimum of 21 years of age. Minimum HS diploma/GED and two (2) years of relevant experience in a behavioral health setting. Must have a fingerprint clearance card issued by the AZ Department of Public Safety or eligible to receive one within eight (8) weeks of hire. TB Test clearance required at the time of hire and annually. Must have reliable transportation, valid AZ Driver's License with state minimum insurance coverage and ability to meet and maintain Copa Health's acceptable driving requirements.
