POSITION SUMMARY
Under the supervision of the Senior Services Manager, provides administrative support for the activities related to the Family Caregiver Support Program, Linkages, and Supportive Services Program contracts.
DUTIES AND RESPONSIBILITIES
Conduct initial intake/screen for potential Care Management and Home-Based Care clients Maintain central intake log to track referrals Complete follow up calls as needed with clients/families/referral sources Document daily activities Provide information and referral Complete clerical duties for all facets of department programming Perform data entry; enter client information into database Prepare and submit weekly, monthly, and quarterly reporting Track, maintain, and report up-to-date list of all active clients monthly to supervisor Assist with audits, client satisfaction surveys, and other documents Minimal front desk duties Related duties, as required
KNOWLEDGE, SKILLS, AND ABILITIES
Experience working with senior citizens highly desirable Excellent verbal and written communication. Bilingual (English/Spanish) Knowledge of MS Office programs (Word, Excel, Outlook)
EDUCATION AND EXPERIENCE
High school diploma; or equivalent; Associate of Arts degree in related field, preferred Prior experience working with seniors and volunteers
LICENSING/CERTIFICATION REQUIREMENTS
Occasional driving is required. Must provide your own vehicle, valid California Driver's License and proof of full insurance coverage. Negative tuberculosis screen (required). We require the selected candidate to undergo a criminal background check using the Live Scan fingerprinting method in compliance with applicable laws and regulations.
WORK ENVIRONMENT/PHYSICAL DEMANDS
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job or that an employee encounters while performing the essential function of this job. Work in moderate noise levels and non-smoking environment Ability to occasionally lift and/or move up to 25 lbs. independently Frequently involves sitting, standing, walking, squatting, bending, or climbing stairs (as needed) Occasionally may be required to stoop, kneel, crouch, or crawl Must be able to speak and hear clearly to communicate effectively Constantly operates a computer, keyboard, and other various office machinery Majority of job duties are performed onsite; however, visits to different sites may be required In compliance with the Americans with Disabilities Act (ADA), reasonable accommodations may be made to qualified individuals with disabilities to perform the essential functions.
COMPENSATION
In accordance with California law, the expected pay range for this position is posted with this position. The actual compensation will be determined based on experience and other factors permitted by law. $21.00 - $24.00 per hour
