ESSENTIAL DUTIES AND RESPONSIBILITIES

• Assist with implementation of member's individual programs, including behavioral supports
• Provide input for interdisciplinary team (IDT) reviews
• Plan and implement daily curriculum
• Implement goals as outlined in the DHSP
• Assist with Activities of Daily Living
• Support members in their personal care, including use of a hoyer lift.
• Support members having difficulty maintaining control
• Teach individuals to perform tasks necessary to function as independently as possible.
• Transport individuals to and from community activities.
• Participate in on-going assessment, modification, and development of program and services.
• Act as a role model, meeting expectations of the community for social norm and dress.
• Complete required trainings and maintain state or agency mandated certifications.

EXPERIENCE

High School Diploma or equivalency. Minimum of at least one year of work experience required in a health care setting.

CERTIFICATES, LICENSES, REGISTRATIONS

Basic First Aid and Adult CPR Valid driver's license and reliable vehicle
