Job Overview Ebenezer is looking for a Adult Day Program Assistant to join our team! This position encourage socialization and assists in planning and implementation of programming needs of individual participants and groups and assist with personal care duties. Responsible for ongoing charting and case management for our Skilled Nursing Facility in Bloomington, MN. This schedule includes; 80 hours per two weeks Day shift 8:00am-4:30pm No weekends Martin Luther Care Center, part of the Martin Luther Senior Living Campus is located in East Bloomington, near woodlands/wetlands and along the Minnesota River Bluffs. Managed by Ebenezer, we offer 137 units between our Skilled Nursing, Memory Care and Transitional Care. We have an outstanding team of tenured staff. The only thing missing is YOU! Martin Luther Care Center was named Top Nursing Home and Transitional Care Center by US News and World Reports the past two years. Responsibilities Assist the Director in the efficient operation of the program: Conducts tours. Meets with families and assists in filling out enrollment paperwork. Communicates with Case Managers and families as needed. Oversees charting of Program Assistants to ensure accuracy and timeliness. Assist with setting up care plans. Conducts quarterly/annual assessments as needed/directed. Assists with office duties (attendance, billing). Functions as Lead staff in absence of Director.
