Job Description

Job description: The expectation of this position is providing direct assistance and supervision of individuals being supported in a 24-Hour Supervised Residential setting. Support includes acquisition, retention, or improvement in skills related to activities of daily living, such as bed making, household chores, education, the preparation of food, therapeutic interventions for managing behavioral and emotional disorders and the social and adaptive skills necessary to enable the individual to reside in a non-institutional setting and become a valuable member of their community. Habilitation, training and instruction are coupled with elements of support, supervision and engaging participation in a therapeutic manner to reflect the natural flow of training, practice of skills, and other activities as they occur during the person's day. This position will provide support to individuals, through Interactions that are designed to achieve outcomes identified on the person-centered plan. Based on the individual served and the goals in their person-centered plan, the level of needs may be either habilitation or therapeutic in nature. This position is under the direct supervision of a Qualified Professional. Essential Duties and Responsibilities 1. Provides training/assistance and supervision to assigned individuals with interpersonal skills and activities of daily living in accordance with the outcomes and goals in the individuals plan (PCP) and under the direct supervision of the Qualified Professional. Address all outcomes and goals as outlined on the individual's PCP. No changes in care are initiated without direction from the Clinical Professional. Documents care per J4 Stability and Safe Haven policies and procedures, evidenced by notes are completed, accurate and turned in on a timely basis, and in accordance with documentation policy guidelines.. Mandatory in-services are attended, verified by sign-in sheet. Support individuals in community participation, including appointments, social activities, and skill-building outings. Utilize de-escalation techniques and follow crisis intervention protocols when needed Maintain accurate, timely documentation, including service notes, incident reports, and progress toward goals Observe and report changes in behavior, mental status, or safety concerns to the QP or supervisor Ensure compliance with HIPAA, confidentiality requirements, and client rights Follow all NC Medicaid, LME/MCO, and agency policies and procedures Participate in required trainings, supervision, and team meetings Minimum Qualifications Proof of High School diploma or its equivalent, and the ability to read and write proficiently in English. Reliable transportation. Current state driver's license. Current CPR card (if applicable). Proof of six months MH/DD experience preferred. Clear criminal background check, acceptable Criminal background record as defined by J4 Stability and Safe Haven Policy and Procedures. Health Care registry check, sex offender registry check and other mandated state registries as defined by J4 Stability and Safe Haven Policy and Procedures Preferred Qualifications Experience working with adolescents with mental health needs Knowledge, Skills, and Abilities Understanding of mental health diagnoses and recovery principles Strong interpersonal, communication, and documentation skills Ability to maintain professional boundaries Cultural competence and sensitivity Ability to work independently while following supervisory guidance Physical ability to perform job duties safely Work Environment Services provided in group home and community settings Will include evenings, overnight, weekends, or holidays Potential exposure to behavioral crises; training and support provided Supervision This position is supervised by a Qualified Professional (QP) as defined by North Carolina Medicaid policy. Services are delivered in accordance with the individual's approved Person-Centered Plan.
