Mental Health Support Specialist Position Summary:

The Paraprofessional provides direct, person-centered support to individuals with mental health diagnoses to promote independence, stability, and improved quality of life. Services are delivered in home and community-based settings in accordance with individualized service plans, agency policies, and applicable state and federal regulations.

Key Responsibilities:

Direct Service Delivery Provide one-on-one support to individuals with mental health diagnoses (e.g., depression, anxiety, bipolar disorder, schizophrenia, ADHD, etc.). Implement strategies and interventions outlined in the Person-Centered Plan (PCP). Assist individuals with developing and maintaining daily living skills (e.g., hygiene, meal preparation, medication reminders, budgeting). Support emotional regulation, coping skills, and behavioral management techniques. Promote community integration by assisting with community outings, appointments, and social activities. Skill Building & Support Teach and reinforce independent living skills. Model appropriate social interactions and communication skills. Assist individuals in identifying triggers and utilizing coping strategies. Encourage goal-setting and progress toward individualized outcomes. Health & Safety Monitoring Monitor for changes in mental status, behavior, or overall well-being. Report concerns such as crisis situations, hospitalizations, or safety risks immediately per agency protocol. Ensure a safe and supportive environment at all times. Documentation & Compliance Complete accurate, detailed service notes in a timely manner (per agency and Medicaid guidelines). Document interventions, individual responses, and progress toward goals. Maintain confidentiality in compliance with HIPAA. Follow all agency policies, procedures, and CARF standards. Collaboration Communicate effectively with Qualified Professionals (QPs), supervisors, and treatment team members. Participate in team meetings, trainings, and supervision as required. Coordinate with families and natural supports when appropriate.

Minimum Qualifications:

High school diploma or GED required. Experience working with individuals with mental health diagnoses preferred. Valid driver's license and reliable transportation. Ability to pass background check and drug screening. Basic computer skills for documentation.

Preferred Qualifications:

Associate's or Bachelor's degree in Human Services, Psychology, Social Work, or related field. Experience with Medicaid-funded services (e.g., CLS, ITS). Knowledge of trauma-informed care and person-centered practices.

Work Environment:

Services provided in homes and community settings May require evenings, weekends, and travel within service area

Physical Requirements:

Ability to stand, walk, and assist individuals as needed Ability to lift up to 25 pounds if required

Supervision:

Reports to Qualified Professional (QP) or Program Supervisor Receives ongoing supervision per agency and regulatory standards

Pay:

$23.00 per hour
